Billing Account Charges

Life and Wellness Center, LLC will bill you through your online Billing Account for services rendered. If you are using insurance, charges will be submitted directly to your insurance provider. You are responsible for any applicable copays or deductibles at the time of service.

Accurate Information

Please provide and maintain current, complete, and accurate information for your Billing Account. Notify us promptly of any changes to your payment method or account details.

Payment Method

Payments will be processed based on your chosen payment method. If we do not receive payment from your provider, you agree to settle any outstanding amounts on your Billing Account upon request.
