Refund and No-show Policy for Patients:

No show fee: $75 

Eligible Refund Cases: A full refund can be requested in the following situations:

  1. Cancellation: If you cancel      your appointment at least 24 hours in advance, you will receive a full refund. Cancellations made within 24 hours of the scheduled appointment      will incur a $50 late cancellation fee.
  2. Provider Cancellation: If your provider cancels or reschedules your appointment prior to      the scheduled time and you choose not to reschedule, you are eligible for      a full refund.
  3. Technical Issues: If technical issues on our side prevent the consultation from      starting, we will make every effort to reschedule your appointment. If      rescheduling is not possible, a full refund will be issued.
  4. Provider No-Show: If your provider is more than 15 minutes late for your      appointment, you may choose to reschedule at the earliest available time, and no no-show fee will apply.
  5. Unavailable Time Slots: If you have paid for your visit and there are no available appointments that accommodate your schedule, you may request a full refund.


Non-Refundable Cases: Refunds cannot be requested in the following situations:

  • Once your appointment has started, no refunds will be issued unless noted in points      3 or 4.
  • External issues on your part, such as internet connectivity problems, are not grounds for a refund.
  • If you fail to show up for your scheduled appointment or do not log in within 5 minutes of the appointment start time, a refund will not be issued. Instead, a $75      no-show fee will apply.


Exceptions:

  • Medical      Emergencies: If you miss your appointment due to an unforeseen medical emergency, you may request a refund. A valid medical document must be provided within 72 hours of the missed appointment.


Refund Process: To request a refund, please contact us at info@lifeandwellnesscenter.com. Include the following information:

  • Full name
  • Date of birth
  • Phone number
  • Date of the      appointment for which you are requesting a refund
  • Detailed reason      for the refund request

Our team will respond within 48-72 hours.


Timeframe for Refund Requests: You have 15 days from the payment date to request a refund if no services have been provided, as outlined in points 1-5. For medical emergencies, requests must be made within 72 hours of the missed consultation.


Refund Processing Timeframe: Refund requests will be reviewed and processed within 2-3 business days. Please allow up to 28 business days for the funds to be credited back to your account.


Policy Changes: Life and Wellness Center, LLC reserves the right to modify this refund policy at any time without prior notice. Changes will be posted on our website and take effect immediately.
